To reduce the geographic barriers to health for all residents, and especially older adults, persons with disabilities, and persons with low-income by improving the coordination of community transportation by convening the SCC 2010 Annual Summit to (1) inform opinion leaders and potential funders about the benefits of coordination to sustain the Regional Transportation Coordination Fund, and (2) provide important technical education programming for those currently involved in regional coordination activities.

To expand diversity within the nursing workforce and nursing education faculty in NH by promoting awareness of the nursing profession among youth from diverse cultures and supporting underrepresented populations to earn a Masters degree in nursing and join the nursing faculty in NH.

To engage existing New Hampshire consumer advocates and the broader stakeholder community--including state leaders and policy makers, providers, small businesses and others--in discussions about the national health reform legislation and likely implementation issues by holding a two-day convening.

To improve access, and the quality and efficiency of the New Hampshire behavioral health system by designing and submitting a 1915b waiver to the Centers for Medicare and Medicaid Services by July 1, 2011.

To support the operations of Health Strategies to implement the Endowment for Health's work as an organization to advance leadership, foster networking and collaboration, enhance knowledge and support the NH safety net.

To enhance the capacity of NH's leaders by providing resources to support national and statewide learning opportunities.

To promote the effective implementation of federal health reform in New Hampshire by providing support to key groups and stakeholders foster collaboration and information sharing and to develop needed knowledge and technical capacity.

To address various cultural and linguistic barriers to healthcare and health-related services and empower community members by providing immigrants and refugees with health education programs presented by healthcare providers and by providing a community health nurse on-site at the Coalition of African Organization's office on a monthly basis.

To foster healthy relationships among African parents and children in Manchester and Concord and increase parental involvement in their children's schools by piloting a culturally appropriate family support model based on national best practices and bridging relationships between schools and refugee families.

To improve outcomes for children with mental health needs who are currently in or are at-risk for out-of-home placement by piloting a high fidelity wraparound approach in Belknap County and by creating a financing mechanism to braid funding across child serving systems.

To provide integrated, family-centered care designed specifically to serve children and families facing the unique challenges presented by homelessness and co-occurring mental illness/substance abuse disorders by reducing common barriers to care faced by vulnerable families.

To ensure quality advocacy, knowledge development and nonprofit capacity building for the mental health system which supports NH's children and their families by sustaining the general operations of key related NH nonprofits.
