To better integrate oral health care of long-term care residents in New Hampshire into general health care by 1) developing a comprehensive oral care training program for health care professionals, 2) establishing an oral health care model that is more coordinated into the care plan system, and 3) developing a sustainable staffing model that utilizes and trains existing nursing home staff.

To ensure quality advocacy, nonprofit capacity building and knowledge development for the health system in NH in order to improve the health of NH residents by piloting the use of core operating support to sustain key NH nonprofits.

To increase support of evidence-based public health legislation and policies in NH by convening an educational conference on public health concepts for NH policymakers (legislators, members of the Executive Council, County Commissioners and Governor's staff).

To maintain access to needed public transportation services in the Lakes Region by assuring a smooth transition between GLTA and the new service provider, supporting a community planning process to oversee future development, and establishing community support for the system

To educate and inform decision makers and legislatures of the health care disparities related to living in rural areas of New England by consolidating existing data and creating educational materials to be used by advocates trying to improve access to health care.

To increase the knowledge of geographic barriers to heath in New Hampshire by analyzing a statewide survey and other related data on tranporation access issues.

To provide high quality current data on NH's livable wage and the availability of livable wage jobs in NH to policymakers and non-profit organizations throughout the state by 1)researching and publishing a new study of NH's livable wage; 2)researching and publishing a new study of the availability of livable wage jobs in NH; and 3)disseminating the results of the two studies widely by hosting forums to present study results, conducting media campaign, and using advisory committee members.

To assure the safety of the members of the Boys & Girls Club of Greater Nashua by providing emergency first aid kits in all appropriate locations.

To increase access to high-quality oral health care for low-income and other under-served populations at the Families First Dental Center by expanding staff time, exploring expansion of physical space, enhancing integration of oral health care with Families First's primary care, prenatal care and other programs; implementing more best practices, and becoming more financially sustainable.

To ensure solvency of NAMI - NH by providing an emergency grant to stabilize their annual budget.

To increase the life expectancy of individuals who have or are at risk of developing Severe & Persistent Mental Illness (SPMI) by providing resources and supports to reduce the risks of hypertension, diabetes, obesity, cardiovascular disease and substance abuse through a variety of community based interventions.

To improve access to preventive and restorative oral health services for all under and uninsured residents of Strafford County by establishing a dental center at Avis Goodwin Community Health Center, integrating medical and dental services by providing education for medical providers on quality oral health assessment, and by establishing a school-based program for elementary children.
