The New Hampshire West-American Red Cross Transportation Program will make it possible for individuals in Cheshire and Western Hillsborough County who do not have access to transportation, to be provided rides to medical appointments.

To improve the mediation program's management system so as to increase the family's access to mediation services.

To build the capacity of government, health and human services leaders, workers and the systems that deliver patient care throughout New England to more effectively address the issue of minority health disparities by holding a bi-annual conference to discuss ways to reduce racial and ethnic health disparities in these workers.

To provide emergency transportation for winter participants in the Adult Day Service Program in order to sustain attendance by providing alternative cost effective transportation.
