To assist New Hampshire's new African communities to achieve complete socio-cultural and economic integration by teaching life skills and health education.

To increase awareness of the importance of healthy eating and exercise for children; to encourage the implementation of good nutrition and physical activity at schools; and to promote policies and practices that will help children to lead more healthy lifestyles, by convening a group of organizations currently involved in children health issues, to assess how NHPTV might work with them, and to plan, execute and evaluate an integrated public information and outreach project.

To improve access to health care services for Somalis living in NH by conducting a needs assessment and engaging and sharing findings with the provider community.

To increase access to emergency contraception for women in New Hampshire by working with key stakeholders to plan for the infrastructure needed to implement and sustain a Collaborative Practice Program; create a training, registration, and monitoring plan for establishing and maintaining collaborative practice agreements for emergency contraception between pharmacists and health care providers; and develop an outreach and awareness program to educate New Hampshire women about the availability of EC.

To improve access to transportation by developing a community-based, collaborative and comprehensive regional transportation plan.

To develop health information exchange capacity in the state by conducting assessment, planning and stakeholder convening.

To improve policy, regulatory and funding factors in order to benefit at-risk seniors and improve long-term care systems primarily in greater Manchester area by better understanding the root causes of public policy barriers, educate and engage stake holders regarding such issues, and develop project strategies to implement change.

To improve access to intensive parenting education, GED program, and medical appointments by providing critically important transportation.

To assure the availability and accessibility of health services to the vulnerable and underserved residents of the service area of Ammonoosuc Community Health Center by sustaining the financial viability of this organization.

To develop a comprehensive state Mental Health Plan.

To increase the availability of health insurance for spouses after divorce by developing awareness of policy options through convening of policy leaders, stakeholders and funders.

To improve the health and well-being of the Antrim, Hancock, Bennington and Francestown communities by maintaining a Time Dollars model - the People's Service Exchange - with participation from a diverse cross-section of the community.
