To create a community health model in New Hampshire to provide better access to prevention and treatment through the use of evidence-based protocols and interventions.

To implement the New Hampshire Oral Health Plan: A Framework for Action.

To develop a sustainable training program for qualified medical interpreter; to provdie leadership to develop a system that meets the interpretation need of the Limited English Proficient (LEP) population statewide; to train and educate health professionals on cultural competency and use of medical interpreters; to provide outreach and education to LEP health consumers about medical interpretation; to develop a continuing education program for training medical interpreters.

To design and implement an Integrated Data system to collect demographic data regarding members and grant applicants of Steppingstones in order to improve data analysis and better respond to indentified needs.

To continue the collaborative development and evaluation of 1) training & education activities and tools and 2) a web-based query system to support NH communities in their efforts to improve the health of their citizens.

To provide state policy-makers, the general public and health system stakeholders with data and information on healthcare finance and coverage by analyzing current status and trends in the health finance system and the impact on the cost and quality of healthcare and on access of residents to that care through health insurance and public programs.

To convene and help fund a one-day conference regarding transportation coordination with national experts, regional and state leaders and developing NH community models.

To develop and analyze information on the physical and mental health care utilization and access for New Hamsphire's foster care children by analyzing existing data and collecting new data from interviews with NH Department of Health and Human Services district office nurses, and a survey of foster parents and focus groups.

To develop a quality infant mental health system in the Upper Valley of New Hampshire.

To address the childhood obesity epidemic statewide by creating healthier schools and communities statewide by addressing issues of nutrition, physical education/activity and school food services in NH schools, grades K to 12.

To advocates for an adequate supply of safe affordable housing in New Hampshire, with special attention to the housing needs of low income people, minorities and seniors.

To conduct and disseminate a comprehensive health and human services needs assessment for Sullivan County.
