Awarded Grants

To create a Center of Excellence for Culturally Effective Care (COECEC)within Manchester Community Health Center by
1-institutionalizing best and model practices of a culturally effective organization (language access, Race Ethnicity and Language (REaL) data , cultural competence, and workforce diversity),
2-creating capacity for leading and teaching quality improvement efforts to address health disparities,
3-and implementing a Community Health Worker (CHW) pilot project with a targeted cohort from two patient populations that experience a higher prevalence of health disparities.

To enhance the capacity of Manchester Community Health Center to serve as a Center of Excellence for Culturally Effective Care, by increasing 1) in-depth knowledge of root causes of health disparities and the current field of research, 2) understanding of cutting edge quality improvement strategies identifying and addressing disparities and 3) leadership skills to implement strategies to transform MCHC and other organizations.

To improve access to health and human services for Bhutanese women and to increase their representation in leadership positions in the community by engaging women in a participatory planning process and leadership development sessions.

To increase the capacity of New American Africans to offer parent leadership and after school programming in Concord and Greater Manchester that will reinforce cultural identity, community strengths-based solutions, and understanding of both concerns and hopes in immigrant and refugee communities by supporting a community planning process and the development of tools/curriculum.

To support a partnership between NAMI NH and the Bhutanese Community of New Hampshire to promote emotional well-being in the Bhutanese community by 1) providing postvention supports to suicide survivors and 2) developing mental health awareness and capacity for suicide prevention within the refugee community.

To expand diversity within the nursing workforce and nursing education faculty in NH by promoting awareness of the nursing profession among youth from diverse cultures and supporting underrepresented populations to earn a Master

To improve the health and well-being of the refugee population in Laconia by 1) conducting health education and outreach, 2) building capacity and a local network of cultural competency trainers in the health care system, and 3) developing a team of volunteers to help address the basic health needs of newly-arrived refugees while promoting self-sufficiency and integration.

To promote health and equity in NH and address health disparities in NH by developing the NH Health & Equity Partnership (H&EP) focusing on strengthening partnerships through advancing leadership, enhancing knowledge, and fostering networking and collaboration.

To advance inclusion and immigrant integration by convening a conference for cross-fertilization and knowledge sharing across New Hampshire's regions, while also offering concrete learning opportunities of best practices for formal and informal helpers who make immigrant integration happen in our state.

To increase access to health care and health for people with limited English proficiency in Belknap and Merrimack Counties by increasing independence and usage of public transportation through outreach to ethnic-based organizations, developing group Travel Training for New Americans, and providing cultural competency training to drivers.

To improve surveillance of work-related injuries and illnesses and understanding of disparities by race, ethnicity and language by implementing a questionnaire with youth and immigrant workers participating in the NH COSH worker safety and health trainings.

To improve health outcomes and access to health care and social services for recent immigrants/refugees with limited English proficiency (LEP) in Laconia by creating sustainable systems to 1) coordinate community resources 2) further integration of refugees within the greater community, 3) recruit and train volunteer advocates to assist with refugee community needs (transportation, medical, and social liaisons) and 4) develop greater cultural competency among volunteers, medical providers and others.
