To improve access to health care among Manchester's youth by developing a process to 1) identify children who do not have health insurance and/or lack a primary care doctor, 2) initiate a health insurance application, 3) help families establish a medical home with an appropriate health care provider, and 4) establish a sustainable process to improve access to health care.
